There are three clues that Sven gives us in his note to help us solve his cypher:

1.  It's a real shift from my usual company around here. 

2. Most of the patients that I play with couldn't tell you what letter a word started with if you write it out in front of them.

3. Always advance from the beginning of the suit.

4. Don't forget, jokers aren't fair play.

First of all this is a shift cypher and the shift starts with the first letter of a certain word. It always starts from the beginning letter of the suit and J=Jack not a joker because jokers aren't fair play. To see how to solve this cypher please visit here.
